The Illusion of Perfect KPIs: Why Are We Still Having LOPCs?

Facilities report zero overdue inspections month after month and still experience loss of primary containment. This article adapts the Objectives and Key Results (OKR) framework used by software reliability teams to mechanical integrity, connecting the daily decisions of individual inspectors to the goal of preventing LOPCs.

Methods to Assess Carburization Damage and Remaining Life in Fired Heater Tubes

Carburization is a common damage mechanism for fired heater tubes in coking service and, left unmanaged, a credible path to loss of containment. It is difficult to inspect for, destructive examination is the usual method, and fitness-for-service practice for it lacks rigorous acceptance criteria. The paper presents an integrity assessment approach that pairs in-service ultrasonic inspection and operating conditions with API 579-1 FFS methods, using empirically determined end-of-life criteria to judge fitness for service and plan tube retirement.
